37d21cea57 Finish up ca-create-cert doc. 
Add in documentation for all the SSL extensions;
Add BUGS, AVAILABILITY, AUTHORS and SEE ALSO sections;
Add comment about email:move to doc for --email option;
Similar misc formatting and whitespace fixes as previous commit;
